Assistance to the disabled individual and/or family with problems involving social and personal adjustment, including health and financial matters; obtaining necessary services through public and private community agencies; assessment of financial, medical, home health care, and counseling needs; and obtaining and coordinating services to meet these needs.
Home and Work Modifications
* Evaluation of an individual’s home and/or work environment using GMR’s Home and Work Accessibility Guide for the Disabled.
* Determination of what modifications in the house or work place are needed.
* Analysis of whether it is more feasible to make changes to the present site or to an alternative setting
* Provide specifications for modification recommendations and the costs of labor and materials required to complete construction.
Provider Identification and Program Placement
GMR’s experience and broad based knowledge in the field is used to evaluate the appropriateness of particular rehabilitative services and programs for the client. We maintain an up-to-date listing of available rehabilitation services, along with a categorical listing of residential and day care programs by features and cost. Each of these is then evaluated based upon the client’s particular needs and requirements. Contacts are established and, if desired, a referral package can be prepared and submitted. Assistance with actual placement can be provided, as well as periodic follow-up to monitor progress and satisfaction with placement service.
An evaluation of the disabled individual is performed to determine the nature, duration and extent of disability in order to plan for and provide medical, vocational, psychological, therapeutic and other services.
